Procedures for Applying for CNEs or Certificate of Completion

Congratulations! You successfully completed the Introduction to Outbreak Investigation course.

You have the option to receive 1.0 contact hours (1.0 clock hours) of CNE credit or to request a non-accredited Certificate of Completion.

CNE credit: There is a $40 fee to receive a certificate from UW Continuing Nursing Education, an ANCC accredited provider. If you wish to apply for CNE credit, print the CNE Application and return the completed form with your payment to:

Certificate of Completion: The Northwest Center for Public Health Practice offers a free, non-accredited Certificate of Completion.

Mail or fax the application with the $40 processing fee to the UW School of Nursing. The address is listed above and on the form.
