Application information for the Management Certificate (Public Health Management Certificate).

Management Certificate Application

Management Certificate applicants must submit all materials by June 3, 2016. For early-bird discounts, materials must be received by May 1, 2016. For more information about the program, contact Janell Blackmer, Management Certificate Coordinator.

Steps to Apply

  1. Compile an application packet with the following:
    • A completed 2016–17 application form
    • A narrative statement (500 words or less) describing a current management goal, what you expect to gain from participating in the program, and how will use the skills you develop during the program
    • A 1–2 page CV or résumé
    • A letter of support from your agency director to Janell Blackmer (you can e-mail it or attach and send with the rest of the application)
    • (If requesting funds for travel) a completed travel assistance form
  2. Submit your compiled application packet and $200 deposit to:

Please note: all applications and deposits must be received by May 1, 2016 to qualify for the early bird rate and June 3, 2016 to qualify for the program.

Payment can be made using a credit card (call Trudy San Jose White at 206.685.2931) or check (made payable to the University of Washington). Applications may be submitted electronically (including electronic signatures or as scanned PDFs of the original printed document) to Trudy San Jose White (sanjose@u.washington.edu).
