Apply for Public Health Management Certificate

Management Certificate applicants must submit all materials by June 1, 2019. For early-bird discounts, materials must be received by May 1, 2019. For more information about the program, contact Janell Blackmer, Management Certificate Coordinator.

Steps to Apply

  1. Compile an application packet with the following:
    • A completed 2019–20 application form
    • A narrative statement (500 words or less) describing a current management goal, what you expect to gain from participating in the program, and how you will use the skills you develop during the program
    • A 1–2 page CV or résumé
    • A letter of support from your agency director to Janell Blackmer (you can e-mail it or attach and send with the rest of the application)
  2. Submit your compiled application packet and $200 deposit to:
