The Northwest Center for Public Health Practice (NWCPHP), Washington State Medical Association (WSMA), Washington State Association of Local Public Health Officials (WSALPHO), Washington State Hospital Association (WSHA), and Association of Washington Public Hospital Districts (AWPHD) are proud to introduce Population Health Meets Medical Care: Breaking the Silos.

Held March 6, in SeaTac, Washington, the conference will bring together executives and leaders working on population health to network and share information, learn about successful local initiatives, and hear from state leaders on new tools and efforts to improve health. Featured breakout sessions will focus on topics such as: mental health, complex health conditions, upstream determinants of health, and at risk populations.
